Male intimate surgery (phalloplasty)

Male genital plastic surgery is a fairly new procedure. Men are mostly (and increasingly) concerned about their penile size. Though men have been known to be preoccupied with their penises, this new generation seems to be particularly fixated on its size
Our clinic’s standard operating procedures of this type include:

  • frenuloplasty (short frenulum of prepuce or tears in the foreskin);
  • the removal of some or all of the foreskin - male circumcision (for hygiene (prophylactic) or ritual reasons)
  • penile deviation treatment (fibroplastic induration of the penis or Peyronie's Disease);
  • testicular implants (endoprosthetic replacement) (cryptorchidism, orchectomy).

Circumcision – circular foreskin excision. The most common reasons for adult circumcision are medical and include the following: phimosis (foreskin is too tight; can't retract from the head), recurrent inflammation of the foreskin (e.g., balanitis, posthitis) or social, cultural, personal, and religious factors. 
Frenuloplasty – surgical approach to lengthening the frenulum of the prepuce when it’s too short. Short frenulum often rip during sexual intercourse: causing pain, bleeding and scarring. It can also be associated with pain during or after intercourse, as well as one of the reasons for premature ejaculation. 
These operations are quite simple, with little trauma and are performed in outpatient facilities under local or general anesthesia. Little postoperative edema and light pain may persist for 2 – 3 days. Patients should abstain from regular sexual activities during 14 postoperative days.
Surgical treatment for the oleogranuloma complication is used to resolve the inflammatory response to vaseline or other oil injections for penile augmentation. The procedure is often quite complicated and its results are not always favorable.
Testicular implants are used for aesthetic reasons to replace missing or removed testicles. They are generally made of a silicone elastomer, have the same weight, shape and pliability of a regular testicle. Testicular implants can often be inserted in less than an hour by a trained surgeon in an outpatient facility. Complications are rare and have no more risk than any other type of implant surgery. Postoperative edema and the feeling of discomfort may last up to 5 days.

Penile Lengthening

According to some research, most men have an erect penis length in the range between 14 to 18 cm, and the average diameter of an erect penis is 2.0 – 2.4 cm . Smaller sizes combined with erectile dysfunction might be the result of endocrine disorders or congenital deformity. Relatively small penis size can cause psychological disorders when the man is quite certain that the reason, for both his and partner’s, sexual dissatisfaction is the penis size. However, too much focusing on the “perceived” problem of size can contribute to erectile dysfunction, thus creating a vicious cycle.
If other non-surgical methods fail to resolve the existing problem or the penis size is too short, operative lengthening can be performed.

The operative procedure can increase the penile length 2.5 – 5 cm and this is accomplished by resecting (cutting) a portion of the suspensory ligaments which hold the penis against the undersurfaces of the pubic bone between the legs. Patients are recommended to use special penile stretching devices (extenders) postoperatively under their doctor’s supervision for 4 – 6 months. The surgery is performed under general anesthesia and last for about 1.5 hours.
This is a relatively minor procedure which is not associated with any extraordinary postoperative complications. Minimal pain is to be expected; most patients complain of discomfort and swelling rather than pain. Normal sexual activity can be resumed after 1.5 – 2 months.

Surgical correction of penile deviation is indicated for treating Peyronie's Disease and other deformities. In this case the operative technique is chosen individually and is case specific.
